Rhabdomyolysis: Symptoms, Causes and Treatment


Rhabdomyolysis is a disorder that involves rapid breakdown of skeletal muscle, due to an injury. As a result the components of the skeletal muscle cells are released into the blood stream. These may include phosphate, potassium and myoglobin. The protein myoglobin is found in the urine. However, destruction of skeletal muscle can increase the myoglobin levels in the urine which can damage the kidneys. The skeletal muscle is a type of muscle in our body, and is made up of muscle fibers. It helps to move the skeleton at joints. The muscles of arms and legs contain skeletal muscle cells.

The kidneys, perform the functions of filtering the blood. But as the blood is overloaded with the byproducts (myoglobin, potassium) of skeletal muscle breakdown, the kidneys may find it extremely difficult to perform its daily task. This can result to kidney failure.

Symptoms

People having rhabdomyolysis often experience pain, swelling and weakness in their arms, legs or lower back. Other symptoms associated with this disease are as follows:
  • Dark-colored urine (abnormal urine color); blood is found in the urine
  • Muscle tenderness; Bruising of muscles
  • Joint pain
  • Fast heartbeats

Causes

Thus, binge drinking on a daily basis is likely to severely damage the skeletal muscles and may even destroy the muscle mass. No wonder, alcoholics face an increased risk of suffering from rhabdomyolysis.

Seizures: As we all know, seizures are characterized by repetitive movement of skeletal muscles. In most cases, the duration of seizures vary from a few seconds to a minute or two. However, if seizure activity continues for prolonged periods of time (more than 5 minutes), it can induce breakdown of skeletal muscles, eventually leading to rhabdomyolysis.

Heatstroke: Doing a physically taxing job for a considerable amount of time in the hot sun increases the body temperature above 104 degree Fahrenheit, a condition commonly referred to as heatstroke. Initially, a person affected with heatstroke experiences muscle cramps and headache but with no treatment and continued exposure to the sun the skeletal muscle fiber may rupture, causing rhabdomyolysis

Medicines: Statins given to high cholesterol patients, may damage the muscles. Statin medications often cause muscle aching. Overdose of diuretics (water pills) or aspirin is harmful, as it causes an electrolyte imbalance. Anti-cholesterol medicines, antidepressants or illegal drugs such as opiates, ecstasy and amphetamines may damage the muscles. Scientists believe that certain cholesterol lowering drugs such as crestor, clofibrate and gemfibrozil, may cause rhabdomyolysis. Crestor has also been linked to renal failure.

Treatment

Once the doctor diagnose the condition, rapid treatment is a must to get a positive outcome. An easy way to prevent life-threatening complications from skeletal muscle disintegration, is to practice generous hydration as early as possible.

Patients are treated with intravenous hydration, where fluids are administered directly into the blood vessels via a catheter (tube). This reduces the harmful side effects of myoglobin. The increase in the amount of fluids due to intravenous hydration, helps the kidneys to get rid of myoglobin.

Blood Transfusion: In this procedure, the patient is given fresh blood via an intravenous line. Blood transfusion is done to replace the blood containing high amount of myoglobin and other substances with healthy blood.

Dialysis: This is a technique in which toxic substances are removed from the blood. This method is used only when the kidneys are not able to do their task of cleaning the blood. The dialysis machine is used to filter the blood. Dialysis may be required for a short period or for the rest of the life depending upon the severity of the condition.

Surgery: Surgical procedure involves cutting the tissues that cover the muscles. This reduces the pressures on the nerves and blood vessels caused by the inflammation of the injured muscle.

Hollow Viscus Injury


The thoracic cavity, abdominal cavity and the pelvic cavity are the three body cavities that make up the ventral body cavity. Though the term 'viscus' refers to a large interior organ which is enclosed in any of these body cavities, this term is mainly used with reference to an organ located in the abdominal cavity. The abdomen refers to the space that lies between the chest and the pelvis. It houses a variety of organs that aid in breaking down the ingested food in a form that can be easily assimilated by the body. The organs that are enclosed within the abdomen include the liver, gallbladder, kidneys, stomach, pancreas, spleen, small intestine and the large intestine. While some of the visceral organs present in the body cavities are solid, some of them are tube-like. The solid organs of the abdomen include the liver, spleen, pancreas and the kidneys. The tube-like organs of the abdominal cavity include the stomach, small intestine and the large intestine. Such a tube-like organ that is located in the abdomen is medically referred to as a 'hollow viscus'. In this article, we will look into the causes, symptoms and treatment of a hollow viscus injury.

Causes and Symptoms of Hollow Viscus Injuries

More often than not, hollow viscus injuries are caused as a result of penetrating trauma to the abdomen. Diagnosis and Treatment of Hollow Viscus Injuries

The treatment options would be suggested once the doctors have conducted certain diagnostic procedures such as an abdominal ultrasound, peritoneal lavage, or a CT scan so as to evaluate the condition of the patient. Surgery is a necessity in case of penetrating trauma, especially if the patient is losing a lot of blood. Doctors may choose to conduct an open surgery or a minimally-invasive laparoscopic surgery. In case of gastrointestinal perforation, wherein the contents of the ruptured viscus have spilled out, the abdominal cavity would have to be cleaned and the perforation would have to be repaired. Urine is the liquid excretory product that contains water and water-soluble waste compounds, generated in the metabolic processes taking place within the living cells. The water-soluble waste material found in urine are mainly nitrogenous products like urea, uric acid, and creatinine. Some hormones, enzymes, and mineral salts can also be found in the urine. Urine is basically a transparent, colorless or pale yellow colored liquid, and is slightly acidic in nature. It's color may fluctuate a little from time to time, and mostly is not a cause for worry.

The characteristic straw or pale yellow color of urine is imparted by the pigment - urochrome. Any change in the color of the urine from pale yellow to dark yellow can be caused due to several factors. We will now have a look at the various causes and related symptoms for this condition.

Causes of Dark Yellow Urine
The color of urine can sometimes serve as an important health indicator. Changes in urine color can be caused by several conditions, ranging from inadequate fluid intake and consumption of some particular foods, to certain serious diseases.

Dehydration
Urine that is dark yellow in color is often a sign that one is dehydrated, or not drinking enough water. This can also happen when an individual sweats a lot, and does not replenish the lost water. Other important factors that can lead to dehydration are excessive vomiting and diarrhea. Due to dehydration, the urine becomes concentrated with nitrogenous waste material, and transforms from a colorless liquid to a dark yellow colored one. A high concentration of nitrogenous waste can give urine a peculiar odor, along with changing its color.

Medications and Supplements
Taking vitamin B-complex supplements can give the urine a dark or bright yellow color. B-complex vitamins are water-soluble vitamins, and hence, any excess amount that cannot be utilized by the body is excreted in the urine. Medications like quinine and quinine derivatives, rifampin, warfarin, nitrofurantoin, and sulfamethoxazole can impart a dark yellow or orange color to the urine too. One can also pass dark urine if he or she has recently used laxatives.

Consumption of Certain Foods
Another common reason behind temporary changes in urine color is the consumption of certain food items. For example, consumption of asparagus can make the urine dark yellow or greenish. Excessive consumption of foods rich in B vitamins, carotene and vitamin C, can also impart a bright to dark yellow color to the urine. On the other hand, consuming a large amount of beets can make the urine reddish-purple in color, while the consumption of blackberries and rhubarb can give it a red tinge.

Diseases and Disorders
Persistent yellow coloration of the urine can be caused by jaundice. Along with urine, jaundice can impart a yellow color to the skin and the sclera of the eyes as well. This change in color is brought about by the accumulation of bilirubin in the blood, which is a waste produced during the breakdown of old red blood cells. Under normal circumstances, the liver removes bilirubin from the blood, where it is combined with bile. This entire process can get disrupted when an infection accelerates the rate at which the red blood cells are broken down, or when the liver itself gets damaged due to an infection or exposure to certain substances.

An obstruction in the bile duct system can also cause jaundice by impairing the movement of bile in the digestive system. Infections of the liver such as hepatitis A, B and C, glandular fever, and leptospirosis, as well as an overdose of paracetamol, liver cirrhosis, autoimmune hepatitis, and liver cancer, can cause jaundice by damaging the liver. Diseases like malaria, sickle cell anemia, thalassaemia, Gilbert's syndrome, etc., can cause jaundice too. Conditions that can obstruct the movement of bile, and thereby cause jaundice include the formation of gallstones, pancreatitis, cholangitis, and cancer of the bile duct and pancreas.

This can result in a decrease of the number of red blood cells in the body. On the other hand, the increased breakdown of the red blood cells can raise the level of bilirubin in the blood, and cause jaundice.
Other Changes in Urine Coloration
Apart from a dark yellow color, the urine can turn orange, brown and greenish, due to several other underlying conditions.
  • The urine may turn dark reddish-brown due to liver diseases like hepatitis and melanoma. It can be an indicator of copper poisoning as well.
  • Smoky brown, and red or pink coloration of the urine can be caused by the consumption of foods like beet, fava beans, and blackberries.
  • Blue or green urine can be associated with certain colors used in food and medicines such as methylene blue. Urinary tract infections and intake of medications like amitriptyline, indomethacin, and doxorubicin, can produce blue- or green-colored urine too.
  • Blue urine can be a symptom of a bacterial infection and 'familial hypercalcemia', which is an inherited condition.
  • Muddy/cloudy and murky urine, along with a bad smell, typically indicates a urinary tract infection.
  • Red or pink urine may indicate the presence of blood in the urine, which commonly occurs due to an infection caused by kidney stones.

Associated Symptoms
Dark yellow urine may not be accompanied by any other symptom, if it is not caused by an underlying disease or disorder.
  • If it is caused by an infection of the urinary tract, one can smell a strong and offensive odor.
  • One may experience the tendency to urinate frequently, and a burning sensation while urinating.
  • An infection can also produce fever and abdominal pain in some individuals.

Before coming to any conclusion regarding the changes in urine color, it is very important to pay attention to the food that has been consumed recently, as well as the amount of fluid intake. This is because, temporary abnormal coloration of urine is commonly caused by the consumption of certain foods, and dehydration. Water plays a key role in draining the impurities out of the body through the urine. When a child passes watery stool for 3 or more number of times in one given day and is accompanied by vomiting, then it becomes a matter of concern for the parents. Vomiting and diarrhea is not a serious issue if it lasts for one or two days. However, if it continues beyond that, then there is a chance of dehydration which can be life-threatening. Basically, vomiting or diarrhea is not a disease in itself. Both of them are symptoms of a number of illnesses that can affect small children. It occurs when the body tries to eliminate some unwanted stuff.

Causes

The most commonly found causes behind vomiting and diarrhea in children is an infection in gastrointestinal tract which triggered by virus. This is also known as gastroenteritis or stomach flu. This virus is highly infectious in nature and it spreads easily from an infected person to others through direct or indirect contact. Other symptoms of this viral infection are abdominal cramps, swollen abdomen, fever, headache and loss of appetite. Treatment

When your child is suffering from vomiting and diarrhea, you must take some quick simple measures at home. Firstly, do not give the child any solid food to eat for 3-4 hours. This will give the much-needed rest to the stomach. Confine the child in one room and ask them to take rest. Use a disinfectant in the bathroom after the child uses the toilet. The most vital step is to replenish the lost fluids and keep the body hydrated. Oral rehydration solution or ORS is highly beneficial for treating stomach problems in children. It can be safely given to babies and even breastfeeding infants. You can get it from local drug stores and grocery stores. Mix the powder in some water and let your child take it in small sips after frequent interval of time. It contains right amount of sugar, salt, potassium and other valuable nutrient which are essential to keep the child nourished and hydrated. In absence of ORS, you can 8 teaspoon of sugar and one teaspoon of salt in 4 cups of water and give it to the child as an alternative.

When there is excessive diarrhea, then you must be extra careful and check out for signs of dehydration. Excessive thirst, dry mouth, low or no urine output, no tears while crying are some of the key symptoms of dehydration in children. In such cases, the child should be hospitalized and intravenous fluids are administered for rehydration. Usually, small children do not require any medicines to stop diarrhea. However, chronic vomiting and diarrhea need some medicinal treatment.

As the vomiting and diarrhea subsides, you can increase the quantity of fluid. Now, you can give other clear fluids like plain water, clear soup, apple juice, etc. For the next few days, bland foods are recommended. It includes banana, rice, applesauce and toast. No fatty foods or dairy products for now as they tend to irritate the stomach and induce diarrhea and vomiting all over again.

Vomiting and diarrhea in children tend to make them extremely weak. So, their body must get sufficient rest in order to recover from it. It should be followed by a nutrient rich diet that can improve their digestive health and prevent recurrence of diarrhea and vomiting.
